The aim of World Hospice and Palliative Care Day ('World Day') was to raise awareness of the need for hospice and palliative care across the world. Your experiences and views of the day are very important and this questionnaire is to help plan future World Days.

Please take the time to complete the questions, there are five sections and it should not take more than 15 minutes. All the information you supply will be treated confidentially and we will share the worldwide results with you.
