Practical guide for acute hospitals on improving end of life care

A practical guide to improving end of life care in acute hospitals has been published by the National End of Life Care Programme.

Transforming end of life care in acute hospitals builds on the framework set out in 'The route to success in end of life care – achieving quality in acute hospitals', published in 2010, and aims to help clinicians, managers and directors implement this framework effectively. The guide uses the service improvement model and processes that underpin the NHS Institute for Innovation and Improvement's 'Productive ward: releasing time to care' series.


The guide emphasises that improving end of life care requires commitment across the trust and its partners and places communication – between professionals, the individual and their carers and relatives – at the heart of care.
