Falls toolkit

Offering hospices the means to manage and prevent falls in their inpatient units in an individualised, multidisciplinary and person-centred way, the toolkit allows hospices to identify falls that are preventable. It also provides the information for falls data analysis.

The toolkit

Help the Hospices ‘Falls toolkit for prevention and management’ was developed by representatives of a group of hospices in 2008.

 

Drawing on a variety of sources in palliative and acute care settings, including, the tool has been informed by existing:

  • policy documents

  • care plans

  • falls assessments

  • accident reports.

 

The tool also builds on:

  • an academic study at Master’s level

  • a falls audit 

  • a benchmarking exercise.

 

The academic study was able to draw heavily on the National Patient Safety Agency observation report 2007 (referenced in the policy) which provided guidelines on falls prevention and management commensurate with the specialist palliative care ethos and practice.

Falls prevention care plan

The care plan aims to reduce the likelihood of falls while maintaining dignity and independence. It  is a template which can be adapted for local use.

Clinical incident report for falls and other incidents

This form has been created to be used to report all patient incidents.
