- care plan
- accident report form.
Falls policy
Defining all patients as at risk of falls, the policy offers a systematic approach to fall prevention and management.
Care plan
The Care Plan has been constructed to put the prevention and management into practice. It is designed for inpatient use, but you could adapt and use it in a community setting.
Accident report form
Record the information needed to decide if falls were preventable.
