3.1    Why do you want to attend this conference?

Tell us why you want to attend hospice23’s residential conference. 

In your answer consider how the knowledge and skills you will learn from the conference will make a difference to:

• you and your professional development

• the team you work in as well as your employing organisation
• the patients and the quality of healthcare provided
• anything else you would like to mention.  

3.2     How will you share what you have learned with others? 

(150 words maximum)

We are looking for evidence that you have thought about how you will share your learning so that more people as well as other organisations benefit from it. For example, in what different ways will you spread what you have learned to your colleagues and other organisations working in your region/country? 

3.3     If the total cost of attending the conference is greater than the maximum amount of bursary available, how will you fund the remaining costs? 

Here we are looking for evidence that you have considered ways of making sure that you can attend the conference, particularly if the total costs more than the maximum bursary available. 

3.4     If your application is successful, are there any further actions required before you can start?  

In the event that your application is successful identify any further actions required before you can start. This could include any problems or things to organise before you can attend the conference.

Section 4: breakdown of costs

• travel and accommodation costs for visiting other palliative care providers whilst in the UK.
  

Provide a detailed breakdown of this total cost. Your application will not be assessed without this breakdown. You can apply for up to £600.

If you have funding secured from other sources, please state how much and from where.
