Nutritional Screening and Assessment in Palliative Care Survey

Thank you for taking part in this important survey, which aims to find out if Hospice inpatient and Day Hospice units are using Nutritional Screening or Nutritional Assessment tools with people who have palliative care needs. 

Definitions

To help you complete the questionnaire, please consider the definitions below.

 

Nutritional Screening

A rapid simple procedure to detect those who have significant nutritional problems or significant risks of such problems in order that clear guidelines for action can be implemented.(1)

 

Nutritional Assessment

A comprehensive evaluation to define nutritional status, including medical history, dietary history, physical examination, anthropometric measurements and laboratory data by a health professional with skills and training in nutrition and nutrition support.
