Hospice care in the community

Hospices provide many different types of care to their local communities. Much of this care is provided in people's own homes or can be accessed by people as out-patients.

Home based care

Hospices meet people's preference by providing the majority of their care in people's own homes.

 

Hospice at Home

Provided by multi-professional teams, hospice at home services allow people to receive hospice care in their own homes; this may be for end of life or respite care.

 

Access to hospice care is especially important to patients and their friends and family when crisis occurs. Therefore, many Hospice at Home services operate 24 hours a day, 365 days a year. 

 

Community nursing

Often led by community nurses, providing specialist care including:

  • advice on pain and symptom control
  • hands on nursing
  • practical advice
  • emotional support.

 

Out-patient care

Hospices also provide services that allow people to remain in their own home for as long as possible.

 

Day therapy

Day therapy gives people the opportunity to spend time in a hospice without necessarily being admitted. The care and support offered includes:

  • medical and nursing care
  • rehabilitation
  • creative therapies
  • complementary therapies.

 

Trends

Responding to changes in demand, hospices have seen substantial growth in services that provide care in settings outside of a hospice building.

 

Hospice at Home services have seen the most substantial growth in recent years.
